Privacy Policy

This policy explains what personal information Usher Care collects, how we use and share it, and the choices you have.
1. Who we are
In this policy, “Usher Care,” “we,” “us,” and “our” mean Usher Care’s affiliated physician-owned medical practice, which provides clinical services, and the company that provides administrative and technology services to that practice.
Usher Care is a healthcare provider. We are not the federal Medicare program, we are not a government agency, and we do not sell insurance.
2. What this policy covers
This policy covers personal information we collect:
- On our website, ushercare.com, including through our contact form.
- Through our ads and lead forms on Facebook and Instagram.
- When you call, text, or email us before you become a patient.
Some information you give us may be protected health information under the Health Insurance Portability and Accountability Act (HIPAA). Where HIPAA applies, our Notice of Privacy Practices also applies. It explains how we use and protect health information and your rights under HIPAA. 3. Information we collect
Information you give us
- Contact details: your name, phone number, and email address.
- Your answers in our forms: for example, who needs help, the age group and state of that person, the kind of help you want, when you want help, the best time to call, whether you’re reaching out as a patient, family member, or professional, your organization, and your message.
- Your consent choices, such as permission to call or text you.
- What you tell us when you call, text, or email us.
If you give us information about another person, such as a parent, please make sure you have their permission.
Please do not put medical details in a form or website message. We will ask for what we need when we talk with you.
Information from Meta
When you submit a lead form on Facebook or Instagram, Meta sends us the information you entered. We also receive the name or ID of the ad and form, the platform (Facebook or Instagram), and the date and time you submitted it. Meta’s privacy policy governs how Meta uses your information.
Information collected automatically
When you visit our website, our website host, Cloudflare, automatically processes technical data such as your IP address, browser type, the pages you request, and the time of your visit. It uses this data to deliver the website and protect it from abuse. We do not use analytics or advertising tools on our website (see section 7).
4. How we use information
We use personal information to:
- Respond to your request and contact you about it.
- Find out whether our services can help you or your family member.
- Schedule calls and services.
- Record your consent and your contact choices.
- Count how many people respond to each ad, in total numbers.
- Improve our services.
- Protect our business and obey the law.
We won’t use your information to market other products or services to you unless you agree.
6. Calls and text messages
If you give us permission, we may call or text you at the number you give us about your request, including with automated technology. Consent is not a condition of any purchase or of receiving our services. Message frequency varies. Message and data rates may apply.
- To stop texts, reply STOP. For help, reply HELP.
- To stop calls, tell us during a call or use our contact form (see section 13).
You can also opt out in any other reasonable way, and we will honor your request within 10 business days. We keep an internal do-not-call list. We don’t include medical details in texts about your request.
No mobile information will be shared with third parties or affiliates for marketing or promotional purposes. All the above categories exclude text messaging originator opt-in data and consent; this information will not be shared with any third parties.
8. How long we keep information
If you do not become a patient, we keep your lead information for 12 months after our last contact with you, then delete it, unless the law requires us to keep it longer. We keep records of consent and opt-out requests for 5 years. If you become a patient, we keep medical records as the law requires. We keep lead information in access-controlled accounts with our service providers.
9. Your choices and rights
You can ask us to:
- Tell you what personal information we have about you.
- Correct or delete it.
- Stop calling, texting, or emailing you.
To make a request, use our contact formand choose “A privacy question or request.” We may need to confirm your identity first. We aim to respond within 45 days. Depending on where you live, state law may give you more rights. We will not treat you differently for using your rights. You can also change your ad settings on Facebook and Instagram.
10. Security
We use reasonable administrative, technical, and physical safeguards to protect personal information. No system is completely secure.
11. Children
Our services are for adults. Our ads and forms are not directed to anyone under 18, and we do not knowingly collect personal information from anyone under 18.
12. Changes to this policy
We may change this policy. We will post the new version on this page and update the effective date. If we make material changes, we will note them at the top of this page.
